Application Process: Degree-Seeking Students

This information is for those who intend to begin the program as degree-seeking students or to apply for formal admission while currently attending as a non-degree seeking student. Please call the University of South Florida Lakeland at (863) 667-7000 for applicable deadlines.


Step 1: Application and Forms

Complete your online application and submit with non-refundable application fee.

Complete your Medical History form and fax to USF Lakeland at 863-667-7096. The Medical History form is required only if you intend to take classes on campus.

Step 2: Student ID


All students must complete a student identification form to obtain a USF Student ID.

Please mail the completed application with a copy of a valid driver’s license, a recent passport photo (minimum 2” x 2”) or 3” x 5” photo (a head shot with a white background), and a $10 check or money order payable to USF to:

You will receive notice of your USF Card ID number by email. Please use this number to complete step 3 in this process.

Step 3: OASIS PIN Number
TO CHECK APPLICATION STATUS:
To check the status of your application, contact Undergraduate Admissions at 813-974-3350 or toll free 877-873-2855, or online through OASIS.

Once you are logged into OASIS, Click Admissions and Financial Aid.
Then click View My Undergraduate Application Status.
The Admission Status site will open in a new browser window
Login with your SSN and OASIS Personal Identification Number (PIN).
Your PIN for your first login is your six digit birth date. Example: 121980
Once you have logged in you will be asked to select a new six digit number for future access.

Please make a note of your new USF Identification Number ex. UXXXXXXXX. You will need this number to complete this application process and for future access to your OASIS student records and registration for classes.

**If you lose your PIN, fax your request to have it reset to 863-667-7096. Your request will be processed within 48 hours.

Step 4: NetID

Obtain a USF NetID. This is necessary to gain access to the USF Portal and your Blackboard class, library privileges, and a host of student benefits. To activate your NetID visit University Network Access.

Then...
  1. Enter the rest of the top number from your USF Student ID Card. This number should start 640013...
  2. Enter your birth date.
  3. Enter your Social Security number.
  4. Click continue.
  5. Follow the instructions for the rest of the screens. When asked to enter a password, it must be from 6-20 characters and cannot contain any word recognizable by a dictionary. It is recommended that you use a combination of numbers and letters.
  6. Verify your email address within your NetID. This should reflect a USF email address. If it does not have a USF email address for you, contact Academic Computing at 866-974-1222.
Step 5: Contact Your Advisor

BSAS with Concentration in Information Technology OR Industrial Operations: Lisa Wetmore 863-667-7064

BSAS with Concentration in Criminal Justice, Early Childhood Development, OR Business: Jacqueline Reece 863-667-7727

Your advisor will work with you to establish a program of study, provide you with appropriate permits after completing the online orientation, and guide you through the university process. You must complete your orientation before you can register for classes. Your advisor will add you to the list for online orientation and contact you with specific instructions.

Step 6: University and Program Information

To complete the online orientation, use the login provided by your advisor.

Step 7: Register for your classes through OASIS

For information about your classes visit the schedule of classes.

You actually register through OASIS.
Registration dates and additional information.

As an approved BSAS student, you should be able to register for your classes after completing your online orientation. But if you are having difficulty, contact your advisor for assistance.

Step 8: You are now officially a registered student

Find your online courses at http://my.usf.edu.
Use your NetID to get into Blackboard.
Click on the courses tab on the top.
The classes you are registered for should be listed on this page.
